Attached below (PDF) is a permission slip for a FREE dental exam for this school year available to ALL students.

Licensed dentists and hygienists will be coming to Mary Morgan Elementary School to examine students.

 

All students who sign-up for this program will receive a dental exam. Eligible students will also receive a cleaning, fluoride treatment and sealants (if needed). Sealants are a protective coating on the chewing surfaces of back teeth.

 

**Please note, this program is for ALL grades but state law requires K, 2nd and 6th graders have a mandatory dental exam prior to May 15th of this school year. This dental exam will fulfill this requirement. State forms will be filled out and left with the nurse's office.

 

The clinic will be held in the old Mary Morgan gym. Transportation will be provided for the Middle School students.

 

A form will be sent home with every child seen explaining what services were performed and if further treatment is needed.

as soon as possible.
