The clinic will begin after school and last for approximately 3 hours, 3:15-6:00 pm. The vaccine/booster clinic will provide the opportunities for interested individuals on a first come, first served basis.

 

1.   Vaccines for children ages 5-11

2.   Vaccines (or Boosters) for children ages 12-17

3.   Vaccines (or Boosters) for 18+

 

All Community members who want to take part in this event are asked to enter in the front of Mary Morgan Elementary School. Below is the the link to the form that participants will need to fill out. It will also be available on-site for individuals to access. All Students under 18 years of age must have a parent with them to participate.

 

This is a community-based clinic, so please feel free to invite anyone else that you feel appropriate.

 

For those receiving their first dose of a vaccine, a follow-up date for the second dose will be February 17, 2022, from 3:15-6:00.
